Fax the Referral Script

Fax the physician's referral script directly to MMR. We require a copy of the script before scheduling treatment. This is the fastest method for most occupational health facilities.

Email a scan or PDF of the referral script to our intake team. Please include the patient's name, diagnosis, and insurance information where available.
